The only things that occur to me are "Miluji Tě navždy" or "Navždy Tvá milující %Name%" I hope someone has more ideas.
Personally I would just engrave it in English. It's a sweet sentiment, it's just that it seems a little akward to say in Czech. Better yet I'd go for French
