when writing czech dates, would it be, for instance, patek, zari 25, 2004? Or patek, 25 zari, 2004? Also, would there be an article in front of the date?

It will be: "pátek 25. září 2004" or "pátek 25. 9. 2004" - no commas, no articles.

    Jana,
Is that how dates are always written, even formally? And would the abbreviation be 29.9.04? Thanks for the help,

Actually, using names of days in writing dates is formal, e.g. put in the headline of a newspaper or in an invitation to a special event. Everyday use prefers just digits, day always first, with a space following the dot (btw, it is a general rule in correct Czech writing, but hardly ever observed), e.g. 30. 9. 04.
