Seems to be a vocative case of opičáček=little monkey

If someone looks like a monkey he or she is not much to look at. The horror, the horror.

You can also be as hairy as a monkey.

(The use of opice suggests ugliness or something unpleasant if it refers to someone`s looks.)

The diminutive (opicacek) takes some weight off all the negativity, but some people could be offended all the same, if unaware of your being sarcastic.
