Requirements for long-term visa, trade license

Hello,

My questions involve the long-term visa and permanent residency requirements.

It states at http://www.mzv.cz/losangeles/en/visa_information/visa_application_form_download/index.html
that all applications "must be made in person, well in advance at the Embassy or Consulate in whose jurisdiction you reside."
Does anyone know if this is a current requirement? Do all family members have to appear for this?

Also, regarding proof of health insurance--are we expected to have US health insurance given that we are attempting to relocate and do not expect to have US legal residences during this time?
