Figuring out a Czech Address

Hello,

I am filling out my student visa application and have run into a problem.
I can't figure out some things regarding the address for my place of residence.
It asks(in separate spots) for Street, Number, Town, psč/zip, State(code), and Telephone number.

I will be at the Pod Palackého vrchem Residence Hall (http://www.skm.vutbr.cz/Angl/PPV.asp?y=1011).

Address: Kolejní 2, 612 00 Brno, Czech Republic

I'm assuming these, please correct me if wrong.

Street: Kolejní
Number: 2??
Town: Brno
ZIP: 61200??
State(code): CZE??

Hi Daniel,

What you have written for the address looks correct. I have seen "STÁT (kód) / COUNTRY (code)" written on another application, so "CZE" is right.
