tř. 5. května = Avenue of May 5th tř. is short for třída (= avenue, boulevard) 5. května = May 5th (Prague uprising against the Nazis) Pečky (www, map) is a town in Central Bohemia. And as for the address, it is this company, right?
You're probably baffled by the fact that the street is named after a date. This is a common occurence in the Czech Republic, though.
